Hi Guys,
Attached is a PDF of a form that the UofU has provided to me. Its a
HIPAA affiliate form in the event we get into HIPAA covered data.
Please fill it out and send it back to me ASAP, you only need to fill
out the fields in the Affiliate section that are text-inputable.
Wrapping up scope discussions with the U in the next couple of days. As
soon as I have stuff I'll put it up on the dump server so you can get at
it and review.
Reminder: I have hotel rooms for Mark, Chuck, and myself. Mark and I
have rooms for Sunday night and Chuck will come down in the morning.
